Protect Protocol Presentation Evaluation Form

Below is the Evaluation Form to fill out for the candidate presenting their Protect Protocol Oral Case Presentation. You may also access the form at this link. Please fill this out with as much detail as possible and make sure to click the submit button at the end. Upon completion, a copy will be emailed to Dr. Grube, Dr. Sefcik and Dr. Ewing. This evaluation form will help us track who is certified in the Protect Protocol (so as to put them on the website) as well as those who did not pass and what items they might need to submit to be able to pass.
